Conditions applicable for Registration of Death:

1. The deceased was a Sri Lankan citizen/dual citizen at the time of death.

2. The death occurred in China.

Documents Required: [Originals + Copy + Death Registration Application Form]

All Photocopies Should Be Submitted in A4 Size.

• Duly filled Form B11 or B15 (late registrations)

• Death certificate of the deceased issued by the competent authority of China

• Passport of the deceased or if the deceased is a minor the passport of the parent in which the endorsement regarding the minor is made.

• Photocopies of (ii) and (iii).

• If Applicable: Cargo receipt/Certification of cremation/Burial documentation.

Application Fee:

For registration of death – RMB 85  USD 13

For issue of death certificate or each additional copy – RMB 85  USD 13

• Payable ONLY by Cash, Bank transfer to the official bank account of the Embassy of Sri Lanka in Beijing

• Personal/Company cheques, Credit Cards, Wechat payments are not accepted.
